This page provides up-to-date information on immunization compliance across the District, in accordance with state health requirements. Transparency in these rates helps families stay informed and supports our ongoing efforts to protect student well-being.

Changes to Oregon Immunization Requirements (effective August 1, 2025)
- A Certificate of Immunization Status (CIS) form with a parent signature documenting the date of chickenpox (varicella) disease will no longer be accepted to document immunity. Schools may accept parent signed history of varicalle disease only if received prior to August 1, 2025.
Acceptable immunity documentation:
- A letter signed by a health care provider with authority to diagnose disease
- A lab report with reference ranges
- A print out from the Alert Immunization Information System (ALERT IIS) showing immunity
- Documentation from a representative of a local health department
- Exclusion Day is moving from the third Wednesday to the fourth Wednesday in February. This gives parents an extra week to provide immunization records or documentation of an exemption or immunity to the school. If the school does not have the required immunization records or documentation by Exclusion Day, the student will not be permitted to attend school.
