Immunization and Medication Requirements

Required Immunizations

The law requires all students (under age 14) entering an Oregon school for the first time be immunized against diphtheria, whooping cough, tetanus (DPT), rubeola (10 day measles), measles, mumps, rubella (MMR), and polio.

Parents must submit proof their children are immunized against the specified diseases. Only medical or religious reasons will exempt children from provisions of the law. School administrators are required by Oregon law to deny admission to children who are not immunized.

Medication at School

Students needing medication during the school day must follow the state law stipulated guidelines:

  • Parents must provide a written request and instructions signed by the child’s doctor before prescription medication can be administered at school. The form is available in the school office.
  • A written request from parents is required before non-prescription medication or any dietary supplement can be administered at school.
  • Parents must deliver the medication to the school office. Students may not bring their own medication.
  • Medication must be in the original prescription container. No envelopes or other containers are permitted. The child will only be given the recommended dosage on the prescription label.
